This role focuses on virtual wellness visits and health assessments for adult and/or pediatric patients, with care delivered in the context of in-home support. The clinician completes preventive screenings, medication reviews, and patient education, while coordinating with in-home team members for vital signs, blood draws, referrals, and documentation. The position emphasizes independent work, strong communication, and use of technology for charting and secure care coordination.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients.
- Provide health risk assessments, medication reviews, and cognitive screenings.
- Coordinate with in-home team members to evaluate vital signs and arrange blood draws.
- Deliver preventive care interventions, patient education, and wellness guidance.
- Manage referrals to specialists and community resources as needed.
- Document clinical findings and care coordination activities in the chart.
- Use laptops, mobile devices, and HIPAA-secure messaging tools for workflow and coordination.
Required
- Board certification as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Louisiana.
- Prior experience performing wellness visits.
- Ability to pass a national background check and valid clinical license search.
Preferred
- Experience working independently in a mobile or technology-enabled care setting.
- Strong comfort with last-minute problem solving and out-of-the-box thinking.
- Prior experience supporting care coordination through secure digital tools.
