Part-time Family Nurse Practitioners provide virtual wellness visits for adult and pediatric patients, focusing on health assessments, preventive care, medication review, and patient education. The work is supported by an in-home clinical team that helps with vital signs, blood draws, and other patient needs. Clinicians also handle care coordination, referrals, documentation, and secure communication using digital tools while working independently in a tech-enabled care model.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients in their homes
- Perform health risk assessments, medication reviews, and cognitive screenings
- Provide preventive care interventions and patient education on health and well-being
- Coordinate with in-home team members to evaluate vital signs, arrange blood draws, and complete patient tasks
- Coordinate referrals to specialists and community resources
- Document findings and manage care coordination using secure digital tools
- Work independently while maintaining high-quality patient care standards
Required
- Board certified Family Nurse Practitioner
- Active Family Nurse Practitioner license in California
- Credentialed Medi-Cal provider in California
- Experience performing wellness visits
- Ability to pass a national background check and clinical license search

- Prior experience working in a virtual or mobile care setting
- Strong comfort with laptops, mobile devices, and HIPAA-secure messaging tools
