The clinician conducts virtual wellness visits and health assessments for adult and pediatric patients in the home, with a focus on preventive care, medication review, cognitive screening, and patient education. The role also supports care coordination, referrals, and documentation, while working closely with in-home team members to complete vitals, blood draws, and other low-acuity tasks. It is a tech-enabled, mobile care model that requires comfort working independently and collaborating across the care team.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients in their homes.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Provide patient education on health, wellness, and preventive care.
- Coordinate with in-home clinical team members to evaluate vital signs, arrange blood draws, and complete patient-specific tasks.
- Manage preventive care interventions, medication management, and referrals to specialists.
- Document findings and support care coordination through secure communication tools.
- Work independently while maintaining communication and collaboration with the broader care team.
Required
- Board certification as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Oklahoma.
- Experience performing wellness visits.
- Strong written and verbal communication skills.
- Ability to work independently or in a team environment.
- Pass a national background check and valid clinical license search.
Preferred
- Experience using laptops, mobile devices, and HIPAA-secure messaging apps for charting and care coordination.
- Prior experience in a technology-enabled or mobile care environment.
- Ability to adapt to changing schedules and last-minute problem solving.
