This role centers on virtual wellness visits for adults and/or children, with a focus on health assessments, preventive care, and proactive care coordination. The clinician also supports medication review, cognitive screening, referrals, and documentation while working with an in-home clinical team that assists with on-site patient needs. The practice uses mobile and HIPAA-secure technology, and the role requires independent work, clear communication, and patient-centered, compassionate care.
Key Responsibilities
- Conduct virtual wellness visits and health assessments for adult and/or pediatric patients.
- Review medications, perform cognitive screenings, and provide patient education on health and well-being.
- Coordinate care with in-home clinical team members to evaluate vital signs, arrange blood draws, and complete patient-specific tasks.
- Provide preventive care interventions, medication management, referrals to specialists, and connections to community resources.
- Document findings and care coordination activities in the clinical record.
- Use laptops, mobile devices, and HIPAA-secure messaging tools for charting and care coordination.
- Work independently while partnering with team members to solve day-to-day clinical and workflow issues.
Required
- Board certification as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Louisiana.
- Experience performing wellness visits.
- Ability to pass a national background check and clinical license search.
Preferred
- Prior experience working in a virtual or tech-enabled care setting.
- Experience with adult and/or pediatric wellness visits.
- Familiarity with care coordination and documentation workflows.
