This role leads a remote team of Nurse Practitioners delivering virtual and home-based care, with a focus on wellness visits, documentation quality, and risk adjustment accuracy. It combines clinical oversight with people leadership, workflow improvement, and cross-functional collaboration across Quality, Compliance, and Operations. The work is tied to value-based care practices and emphasizes high standards for HCC coding, MEAT documentation, and team development.
Key Responsibilities
- Lead and coach a remote team of Nurse Practitioners across multiple states.
- Review schedules, visit volumes, performance dashboards, and escalations.
- Oversee wellness visit quality, documentation accuracy, and compliance with HCC and MEAT standards.
- Provide regular feedback, 1:1 support, and development guidance to team members.
- Coordinate with Quality, Compliance, and Operations to improve workflows and care delivery.
- Monitor documentation quality through audits and reviews.
- Plan training and process improvements to strengthen clinical quality and team engagement.
Required
- Board Certified Nurse Practitioner.
- Active and unrestricted Nurse Practitioner license.
- 5-7 years of clinical NP experience.
- At least 2 years of direct people leadership experience.
- Strong knowledge of HCC coding, risk adjustment, and MEAT documentation standards.
- Experience using EMRs, dashboards, and communication tools.
Preferred
- Multi-state or compact state Nurse Practitioner license.
- Experience in value-based care, home-based care, or virtual primary care.
- Certified Risk Adjustment Coder (CRC) or similar credential.
- Experience leading remote teams in a high-growth environment.
