In this remote RN advocate role, the clinician guides patients through complex health concerns using video and phone visits, helping them understand their care and work toward health and wellness goals. The work centers on care coordination, problem-solving, and support around social determinants of health such as food access, transportation, and help at home. The role uses Solace’s proprietary tools and workflows and emphasizes building trusting patient relationships.
Key Responsibilities
- Conduct video and phone-based patient interactions to support navigation of complex health concerns.
- Build individualized care plans that prioritize patient needs and support consistent follow-through over time.
- Coordinate support for social determinants of health, including food resources, transportation access, and in-home support.
- Use Solace’s proprietary systems, tools, and technology in day-to-day patient advocacy work.
- Provide empathetic, action-oriented guidance to help resolve patient problems.
- Build trusting relationships through active listening and ongoing communication with patients.
- Balance multiple tasks while working under pressure in a fast-moving care environment.
Required
- Active Registered Nurse (RN) license in good standing.
- Experience working with diverse patient populations.
- Ability to work remotely from the United States.
Preferred
- Experience learning and adapting to new technology platforms quickly.
- Strong organizational skills in a patient-facing care coordination role.
- Prior experience supporting complex or underserved patient needs.
