Provide telehealth outpatient nutrition counseling for patients with chronic health concerns in an integrative and functional nutrition setting, with a focus on personalized, root-cause care. Visits emphasize metabolic health, digestive disorders, hormonal imbalances, and other chronic conditions through nutrition counseling, education, and health coaching. This PRN role offers flexible scheduling with consistent weekly availability, and may include collaboration with a clinical manager and credentialed care workflows.

Key Responsibilities

Conduct virtual nutrition counseling visits for outpatient patients with chronic and complex health needs.
Provide personalized medical nutrition therapy using integrative and functional nutrition approaches.
Educate patients on nutrition, lifestyle change, and sustainable habit formation.
Support care plans for metabolic health, digestive disorders, hormonal imbalances, and other chronic disease conditions.
Coordinate care within an insurance-funded, telehealth-based practice model.
Document patient encounters and progress in alignment with clinical workflows.
Collaborate with clinical leadership and the broader care team as needed.

Required

Registered Dietitian or Registered Dietitian Nutritionist (RD/RDN) credential.
Current CDR registration.
At least 2 years of experience in outpatient or clinical nutrition care.
Pennsylvania licensure or eligibility for licensure.
Minimum of 12 hours of weekly availability.
Ability to obtain major insurance credentialing before start.

Preferred

Experience with evening appointments.
CDCES certification.
IFNA certification or IFNCP credential.
Enrollment in IFNA training.
Background in integrative or functional nutrition.
