Provide remote care navigation and advocacy for older adults on Medicare, helping patients and families manage mental health needs, medical system challenges, and day-to-day barriers to care. Work centers on individualized support that addresses social determinants of health, including transportation, food and housing resources, medication reminders, scheduling, and caregiver education. You will collaborate with therapists, care coordinators, and a tech-enabled platform to deliver person-centered support.
Key Responsibilities
- Build trusted relationships with older adults and their families through empathetic listening and guidance.
- Serve as a resource for navigating medical systems, social services, and emotional wellness needs.
- Create individualized care plans that address clinical needs and social determinants of health.
- Coordinate access to food and housing programs, transportation, appointments, and follow-up care.
- Support medication management, reminders, caregiver education, and ongoing patient engagement.
- Collaborate with therapists, care coordinators, and the tech platform to support seamless care delivery.
- Provide feedback to help shape the patient advocacy model as the program scales.
Required
- Active Registered Nurse license in good standing.
- Experience in healthcare advocacy or care navigation with Medicare populations.
- Familiarity with social determinants of health and their impact on patient outcomes.
- Based in the United States.

- Experience working remotely and using digital tools or new care platforms.
- Strong organizational, communication, and multitasking skills.
- Clinical judgment paired with empathy and problem-solving.
- Experience supporting older adults with mental health or complex care needs.
