The RN provides after-hours and weekend telephone triage support for residents in assisted living communities, using clinical assessment skills to guide care decisions and delegate to resident assistants as needed. This role focuses on older adults and requires comfort supporting gerontological, rehabilitative, and restorative care needs by phone. It is a hybrid/remote-friendly position that may office from a PHS location or from home, with weekend coverage.
Key Responsibilities
- Provide over-the-phone clinical assessment and triage for assisted living residents.
- Respond to calls from assisted living communities during after-hours and weekend coverage.
- Delegate and communicate care guidance to resident assistants/home health aides.
- Support regulatory and clinical practices that affect resident care decisions.
- Use computer systems and documentation workflows to manage triage requests.
- Coordinate with community staff to help maintain high-quality resident care.
Required
- Current RN license in Minnesota, Wisconsin, and Iowa.
- Registered nurse with current licensure in the states where PHS operates.
- Ability to read, write, speak, and understand English effectively.
- Residence in Minnesota, Wisconsin, or Iowa.
- Assisted living nursing and/or phone triage experience.
Preferred
- Interest in gerontological nursing.
- Interest in rehabilitative and restorative clinical practices.
- Compatibility with PHS mission and operating philosophies.
