The clinician provides virtual primary care for members with chronic conditions, using a functional and integrative medicine approach and personalized treatment plans. Care is delivered through telemedicine for a health-focused, tech-enabled practice, with close coordination with a care team and health coach. This part-time contract role also includes handling patient inquiries, documenting visits, and following practice protocols.
Key Responsibilities
- Conduct virtual member visits and related administrative work on a weekly basis
- Provide personalized primary care for patients with chronic conditions using a root-cause, functional medicine approach
- Work closely with care team members, including health coaches, to support patient outcomes and wellness goals
- Assist with patient inquiries and requests in a timely manner
- Complete online charts accurately and on time
- Use the practice’s telemedicine platform to deliver care
- Follow organizational policies, procedures, and clinical protocols
Required
- Active MD or DO license
- Board certification in internal medicine, family medicine, obstetrics/gynecology, rheumatology, endocrinology, or another primary care field
- Active medical license in at least one of the listed states
- 3+ years of primary care experience, either via telemedicine or in person
- 2+ years of clinical practice after residency
- 2+ years of functional medicine experience
Preferred
- Active medical license in one of the additional preferred states
- Familiarity with a MacBook, Google Suite, Slack, and Zoom
