This physician role focuses on functional medicine visits for adults with chronic conditions, using advanced testing and personalized care to identify and address root causes. Care is provided both virtually and in person, with scheduled time for patient communication and ongoing relationship-building over time. The role also includes collaboration with health coaches and care teams, plus opportunities to contribute to research or technology improvements.
Key Responsibilities
- Conduct functional medicine visits virtually and in person.
- Use advanced diagnostics and testing to evaluate root causes of chronic illness.
- Provide ongoing primary care for members with chronic health needs.
- Partner with health coaches and care teams to support patient outcomes.
- Document care in a proprietary electronic medical record system.
- Communicate with patients outside of scheduled visits as part of the care model.
- Participate in additional projects such as health outcomes research or technology improvement.
Required
- MD or DO degree with an active medical license.
- Active New York medical license.
- Board certification in internal medicine, family medicine, or another primary care field.
- At least 3 years of primary care experience in telemedicine or in-person settings.
- Experience as an experienced, board-certified physician.
Preferred
- Experience practicing functional medicine.
- Formal functional medicine training, such as IFM training.
- Additional medical licenses in states including CA, TX, MA, NJ, IL, PA, VA, NC, CO, GA, AZ, WA, or CT.
- DEA license.
- Familiarity with MacBook, Google Workspace, Slack, and Zoom.
- Experience working in a fast-paced, high-growth, tech-enabled environment.
