Provide functional medicine care for patients with chronic illness through longer visits, using advanced diagnostics, lifestyle intervention, and individualized treatment plans. Care includes both virtual and in-person visits, with at least three days per week seeing members in person and ongoing collaboration with a multidisciplinary support team. The role emphasizes root-cause, preventive, and whole-person care in a tech-enabled practice model.
Key Responsibilities
- Conduct functional medicine visits virtually and/or in person
- Use advanced testing to identify root causes of chronic illness
- Provide personalized treatment plans centered on lifestyle medicine, nutrition, and long-term care
- Partner with health coaches, RN care managers, and other clinicians to support outcomes
- Build ongoing patient relationships through extended visits and follow-up care
- Document care in a proprietary electronic medical record
- Work within evidence-based protocols and tools to support clinical decision-making
Required
- Board-certified MD, DO, NP, or PA
- Active state licensure in New York or California
- At least 3 years of post-residency clinical experience
- Experience or training in functional medicine preferred by the employer
