Provide virtual, synchronous and asynchronous consultations for older adults, with a focus on reviewing medical history, identifying health-related social needs, and creating care plans that address both clinical and social needs. Care is delivered through a physician-led, tech-enabled telemedicine model and is supported by Community Care Advocates who help implement the plan. This role also includes chart review, escalation support, and input into workflow design as part of an early physician team.
Key Responsibilities
- Conduct virtual visits and review patient medical histories for older adults.
- Identify health-related social needs and incorporate them into care planning.
- Create and update care plans for Community Care Advocates to carry out community support services.
- Participate in asynchronous chart reviews and clinical escalations as needed.
- Collaborate with operational and partner teams to refine workflows and improve the care model.
- Provide input that helps shape clinical processes and address operational challenges.
Required
- Active, unrestricted MD or DO license in good standing.
- Board certification in Internal Medicine or Family Medicine.
- Active Florida state medical license.
- Florida Medicaid credentialing and Medicare enrollment.
- At least 2 years of post-residency clinical experience.
- Ability to work a minimum of 8 hours per week for weekday synchronous consultations.
Preferred
- Prior telemedicine experience.
- Additional state licensure or credentialing.
- Fluency in Spanish.
