Provide telephonic and telehealth urgent care for patients, caregivers, and facility staff across 24/7 after-hours coverage, with a focus on acute, chronic, and behavioral health needs. This role also supports care coordination across settings, including transitions between facilities, hospitals, primary care, and specialty care, while using video visits when clinically needed. It is a virtual, patient-facing advanced practice role that emphasizes documentation, collaboration, and evidence-based care.
Key Responsibilities
- Conduct telephonic and telehealth visits to assess and manage urgent care needs.
- Provide direction to patients, caregivers, and facility staff during after-hours coverage.
- Coordinate care across care settings, including hospitals, nursing facilities, primary care, and specialty providers.
- Manage acute, chronic, medical, and behavioral health conditions in collaboration with physicians or specialty providers.
- Document assessments, encounters, quality elements, and diagnoses in the EMR in compliance with CMS and documentation requirements.
- Review orders, interventions, and response to treatment to support the most effective plan of care.
- Participate in advanced care planning conversations with members and families.
- Use evidence-based practice guidelines and complete required training and quality initiatives.
Required
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
- Active and unrestricted NP license in the state of residence and in New York.
- Board certification as a Nurse Practitioner through AANP or ANCC.
- Certification in Family, Adult, Gerontology, or Adult-Gerontology Acute Care Nurse Practitioner.
- Current active DEA licensure or prescriptive authority, or ability to obtain it post-hire where allowed.
- Ability to work scheduled shifts, including evenings, nights, weekends, and holidays.
- Proficient computer skills and ability to document in electronic medical records.
- Driver’s license and access to reliable transportation.
Preferred
- 3+ years of clinical experience as an advanced practice clinician.
- Active and unrestricted license in additional states such as North Carolina, Georgia, Alabama, Mississippi, or South Carolina.
- Experience caring for patients with complex behavioral, social, or functional needs.
- Experience in a nursing home or with seniors in an acute care facility.
- Understanding of geriatrics, chronic illness, acute disease management, advanced illness, and end-of-life discussions.
