Provide telephonic and telehealth urgent care for patients across home and community settings, with after-hours coverage for evenings, nights, weekends, and holidays. This role focuses on acute, chronic, medical, and behavioral needs while coordinating with caregivers, facilities, primary care providers, specialists, and field staff. The position is fully remote and emphasizes interdisciplinary care, documentation quality, and evidence-based practice.
Key Responsibilities
- Conduct telephonic and video visits to assess and manage urgent care needs.
- Provide direction and care coordination for patients, caregivers, facility staff, and other clinicians.
- Manage medical, behavioral, chronic, and acute conditions in collaboration with physicians or specialty providers.
- Perform comprehensive assessments and document findings in the EMR in compliance with CMS and quality requirements.
- Coordinate transitions of care across settings, including hospitals, facilities, and home-based care.
- Review orders and interventions for appropriateness and response to treatment.
- Address advance care planning discussions with members and families.
- Participate in quality initiatives and required education/training.
Required
- Master of Science in Nursing or Doctor of Nursing Practice from an accredited program.
- Active and unrestricted Nurse Practitioner license in the state of residence.
- Active and unrestricted Nurse Practitioner license in New York, New Jersey, Connecticut, Maine, New Hampshire, or Rhode Island.
- National board certification as a Nurse Practitioner.
- DEA licensure or prescriptive authority, or ability to obtain it post-hire as allowed by state regulations.
- Ability to work scheduled shifts, including required after-hours, weekend, and holiday coverage.
- Proficiency with computers and electronic medical record documentation.
Preferred
- 3+ years of clinical experience as an APC.
- Additional active and unrestricted licensure in New York, New Jersey, Connecticut, Maine, New Hampshire, or Rhode Island.
- Experience caring for patients with complex behavioral, social, or functional needs.
- Experience in nursing homes or acute care settings serving older adults.
- Background in geriatrics, chronic illness, acute disease management, or advanced illness discussions.
