This role provides home health nursing care in patients’ homes, focusing on assessment, care planning, treatment, and ongoing monitoring for a broad range of post-acute and chronic needs. The RN works independently in the field while coordinating closely with the care team to support safe, timely, patient-centered care.
Key Responsibilities
- Provide skilled nursing care in the home setting.
- Conduct patient assessments and monitor changes in condition.
- Develop, implement, and update individualized care plans.
- Coordinate care with physicians, therapists, and other members of the interdisciplinary team.
- Document visits, interventions, and patient progress accurately.
- Educate patients and families on treatment plans, medications, and self-care.
Required
- Current RN licensure.
- Experience or training in home health or a related nursing setting.
