This role centers on managing and supporting home health nursing care for adult and medically complex patients, with a focus on clinical oversight, care coordination, and day-to-day team guidance. It also involves broader operational responsibilities such as documentation review, quality monitoring, and collaboration with clinicians and other care team members to help ensure safe, consistent care delivery.
Key Responsibilities
- Provide clinical oversight for nursing care plans and patient visits
- Coordinate care with interdisciplinary team members and outside providers
- Review documentation, plans of care, and clinical outcomes for quality and compliance
- Support and supervise nursing staff in daily workflow and patient care needs
- Participate in patient/caregiver education and discharge planning as needed
- Use home health processes and electronic documentation systems to support care delivery
Required
- Active RN license
- Clinical nursing experience in a patient care setting
- Experience in home health or related community-based care preferred for this type of role
