This role provides skilled home health nursing care for patients in the home setting, with a focus on assessment, care planning, treatment, and ongoing monitoring. The nurse supports patients with a range of medical needs and coordinates closely with the care team to help manage recovery and prevent complications.
Key Responsibilities
- Conduct home health assessments and develop individualized nursing care plans.
- Provide skilled nursing interventions, treatments, and ongoing patient monitoring in the home.
- Coordinate care with physicians, therapists, and other members of the interdisciplinary team.
- Document patient status, services provided, and care plan updates accurately and timely.
- Educate patients and caregivers on self-care, medications, and symptom management.
- Participate in care coordination and follow-up to support safe transitions and continuity of care.
Required
- Registered Nurse (RN) licensure.
- Experience in home health or a related clinical nursing setting.
