This role provides in-home nursing care for patients receiving home health services, with a focus on assessment, treatment, and ongoing monitoring in the patient’s residence. The clinician supports care coordination and documentation while working closely with the broader care team to help patients manage health needs outside the hospital setting.
Key Responsibilities
- Conduct skilled nursing visits in patients’ homes
- Provide assessments, monitoring, and ongoing evaluation of patient status
- Coordinate care with physicians, therapists, and other members of the home health team
- Document visits, care plans, and patient progress in the medical record
- Educate patients and caregivers on treatment plans and self-care needs
- Monitor changes in condition and escalate concerns as needed
Required
- Registered Nurse licensure
- Experience in home health or a related nursing setting
