This role focuses on providing skilled nursing care in the home setting for patients in the Kingston area. The clinician will assess patient needs, deliver treatment, and coordinate care across the home health team.
Key Responsibilities
- Conduct skilled nursing visits in patients’ homes.
- Assess patient status, monitor progress, and update care plans as needed.
- Provide treatment, education, and support for patients and families.
- Coordinate care with physicians and the broader interdisciplinary team.
- Document clinical findings, interventions, and patient outcomes.
Required
- Current Registered Nurse (RN) license.
- Experience or training in home health nursing.
