This role focuses on providing skilled nursing care in the home setting, with attention to patient assessment, care planning, and ongoing monitoring. The clinician works with patients and families to support recovery, manage symptoms, and coordinate care across the broader interdisciplinary team.
Key Responsibilities
- Conduct comprehensive nursing assessments in the patient’s home.
- Provide direct skilled nursing care and ongoing clinical monitoring.
- Coordinate care plans with physicians and other members of the care team.
- Document patient status, interventions, and progress in the medical record.
- Educate patients and caregivers on treatment plans, medications, and self-care.
- Monitor for changes in condition and escalate concerns as needed.
Required
- Active Registered Nurse licensure.
- Current clinical nursing experience.
