This is a remote, part-time physician pathways role for a doctor in the final stage of training who wants early exposure to value-based, population health-focused care. The program offers mentorship and practical insight into primary and specialty care within a patient-centered medical home model, with opportunities to learn across different care settings. It is designed to help transition from residency or fellowship into practice while working only a few hours per month.
Key Responsibilities
- Participate in a customized physician pathways program during the final year of training.
- Gain exposure to primary care and specialty care workflows across multiple care settings.

- Collaborate with experienced physicians for mentorship and transition support.
- Engage with clinic, surgery center, urgent care, and virtual practice environments.

Required
- Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.).
- Transitioning into the final year of residency or fellowship, or early in the final year.
