Key Responsibilities
- Provide palliative care assessments for patients with serious or advanced illness.
- Manage symptoms and help optimize comfort-focused treatment plans.
- Coordinate care with physicians, nurses, and other members of the interdisciplinary team.
- Support goals-of-care discussions and advance care planning.
- Document clinical findings, care plans, and patient progress.
Required
- Certified Nurse Practitioner credential.
- Advanced practice nursing licensure in the applicable state.
- Experience in palliative care, hospice, or a related serious-illness setting.
