This role focuses on in-home, patient-centered primary care for adults with chronic and acute needs, with an emphasis on managing common medical conditions in the home setting. The clinician conducts visits, evaluates ongoing health concerns, and helps coordinate care for patients who may benefit from a more accessible alternative to clinic-based treatment.
Key Responsibilities
- Conduct home-based patient visits and assess acute and chronic medical concerns.
- Provide primary care management for adults in their homes.
- Coordinate ongoing care and follow-up for patients with complex access or mobility needs.
- Document clinical findings and care plans in the medical record.
- Collaborate with other members of the care team as needed to support continuity of care.
Required
- Active Nurse Practitioner licensure or eligibility to practice as a Nurse Practitioner.
- Experience providing direct patient care in a clinical setting.
