Key Responsibilities
- Conduct patient assessments and follow-up visits in the home setting
- Provide evaluation and management for patients needing care at home
- Coordinate care plans with other members of the care team
- Use clinical judgment to address ongoing patient needs outside the clinic
Required
- Current licensure as a Nurse Practitioner or Physician Assistant
- Ability to provide care in a home-based clinical setting

- Experience with home-based care or care-at-home models
