Key Responsibilities
- Coordinate inpatient care management activities for hospitalized patients.
- Support utilization review and care planning workflows.
- Communicate with clinical teams to help align services and discharge planning.
- Review patient needs and assist with appropriate transitions of care.
- Document care management actions and clinical coordination activities.
Required
- Active RN license.
- Experience in inpatient care management or a related acute care setting.
- Ability to work remotely while supporting Georgia-based patient care workflows.
Preferred
- Experience with utilization management or case management.
- Background in hospital-based nursing or discharge planning.
