This role focuses on providing hospice nursing care and case management for patients with serious or terminal illness. The clinician coordinates visits, assesses symptoms and needs, and supports patients and families across the end-of-life care journey. Work is patient-centered and requires close collaboration with the broader care team.
Key Responsibilities
- Conduct hospice nursing assessments and ongoing patient visits
- Coordinate and manage care plans for patients and families
- Provide symptom management and support for end-of-life needs
- Collaborate with interdisciplinary team members to align care
- Document clinical findings, care plans, and visit updates
- Support patient and family education related to hospice care
Required
- Registered Nurse (RN) license
- Experience in hospice, palliative care, or case management
