This role focuses on providing skilled home health nursing for patients in their homes, with day-to-day care centered on assessment, treatment, and ongoing monitoring. The work is primarily in-home and may include coordination with other members of the care team as needed.
Key Responsibilities
- Conduct in-home nursing assessments and evaluate patient status.
- Provide skilled nursing care, treatments, and education in the home setting.
- Coordinate care with physicians and interdisciplinary team members.
- Monitor patient progress and update care plans as needed.
- Document visits, interventions, and patient responses in the clinical record.
Required
- Registered Nurse (RN) licensure.
- Experience in home health or a related clinical setting.
