Provides skilled home health nursing for patients in their homes, with a weekend schedule focused on assessing needs, developing care plans, and delivering ongoing clinical support. Work is centered on home-based patient care and may include coordination with interdisciplinary team members and communication with providers as needed.
Key Responsibilities
- Conduct home health assessments and monitor patient condition in the home setting.
- Provide skilled nursing interventions, treatments, and ongoing clinical support.
- Coordinate care plans with the interdisciplinary team and other providers.
- Document visits, findings, and patient progress in the medical record.
- Participate in weekend coverage and follow-up care as assigned.
Required
- Current Registered Nurse (RN) licensure.
- Experience or training in home health nursing.
Preferred
- Experience working weekend schedules.
- Prior experience coordinating care in a multidisciplinary setting.
