This part-time contract RN role focuses on clinical chart review and medication support for patients with cardio-kidney-metabolic conditions. The nurse reviews records to help determine fit for the app, reconcile discrepancies across sources, and identify medication-related issues such as adherence concerns, duplication, or gaps.
Key Responsibilities
- Conduct chart reviews to assess patient fit for the app using diagnoses, prescription activity, test results, and medical records from multiple sources.
- Reconcile discrepancies across clinical documentation and medication information.
- Review medication lists for completeness, appropriateness, adherence concerns, duplication, and gaps.
- Support medication management workflows that do not include prescribing.
- Coordinate with patients’ clinical teams about opportunities to improve medication experience.
- Support the Medical Director with program and process development for patient care.
- Assist in designing future clinical tasks and responsibilities.
- Participate in product feedback and quality improvement efforts.
Required
- Current RN licensure in Illinois.
- Experience in cardio-kidney-metabolic conditions.
- At least 6 months of clinical chart review experience.
- Comfort learning new technology.
Preferred
- RN licensure in California and/or Tennessee, or a Compact license.
- Experience with cardio-metabolic care.
- Interest in digital health and informatics.
- Experience with risk adjustment review of medical records.
