This role focuses on pharmacy pathway development and clinical decision support for a value-based care population. The clinician will use medication data, clinical guidelines, and regulatory requirements to support prescribing decisions, generic substitution, and management of high-cost patients, polypharmacy, and medication optimization. The work is largely cross-functional and includes supporting internal providers, case managers, and governance processes.
Key Responsibilities
- Collaborate with medical leadership to develop and manage pharmacy pathways for multiple health conditions.
- Develop clinical decision support content, including dosing adjustments, contraindications, interactions, side effects, special populations, and allergy considerations.
- Maintain and update the pharmacy database in the clinical decision support tool.
- Support dashboard workflows that educate providers and align prescribing with pathway guidance.
- Analyze high-cost patient data with clinicians and case managers to improve medication use, including generic drug use and polypharmacy reduction.
- Develop and maintain formulary guidance and generic-versus-brand recommendations.
- Support clinical governance by documenting guideline rationale, version history, evidence sources, and review schedules.
- Contribute to training materials, user guides, clinical documentation, and staff education as needed.
Required
- Doctor of Pharmacy (PharmD) degree.
- 10+ years of professional experience.
- At least 5 years of experience developing and/or managing clinical programs, products, or services.
- Working knowledge of clinical guidelines.
- Experience managing cross-functional teams on large-scale projects.
Preferred
- Experience in clinical program management, clinical guideline development, clinical compliance, utilization management, case management, or clinical research.
- Experience in a highly regulated environment with federal, state, and accreditation standards.
- Experience managing dependencies across teams in a decentralized business structure.
