Provides nursing support for members enrolled in a self-directed personal care program, with a focus on helping coordinate and monitor care needs in the community. The role centers on assessing member needs, supporting person-centered care planning, and working with members, caregivers, and service teams to help maintain safe, appropriate care at home.
Key Responsibilities
- Conduct nursing assessments and monitor member care needs in the self-directed personal care program.
- Coordinate with members, caregivers, and support teams to help develop and update care plans.
- Provide guidance related to personal care services and safe care delivery in the home setting.
- Participate in ongoing review of member status and care concerns.
- Use documentation and care coordination workflows to support continuity of services.
Required
- Active Registered Nurse (RN) license.
- Experience or training relevant to community-based or home care nursing.
- Ability to work with members receiving long-term support services.
