This RN role focuses on supporting adults enrolled in a self-directed personal care program by assessing needs, coordinating care, and helping ensure services are delivered safely and appropriately. Key Responsibilities
- Conduct nursing assessments for members receiving self-directed personal care services.
- Coordinate care plans and service delivery with members, families, and care teams.
- Monitor member needs, safety concerns, and changes in condition.
- Document assessments, care coordination activities, and service recommendations.
- Support appropriate use of personal care services within the program guidelines.
Required
- Active RN license.
- Experience in nursing care coordination, home health, community health, or a related setting.
- Ability to work with adult members requiring personal care support.
