This role focuses on nursing assessment and care coordination for members enrolled in a self-directed personal care program, with an emphasis on supporting daily living needs and health status monitoring. Key Responsibilities
- Conduct nursing assessments for members receiving self-directed personal care services.
- Coordinate care plans and service needs based on member assessments and ongoing status changes.
- Provide oversight related to personal care supports and member safety in the community setting.
- Communicate with members, caregivers, and other care team members to support service delivery.
- Document assessments, care coordination activities, and updates in the appropriate systems.
Required
- Registered Nurse (RN) licensure.
- Experience or background relevant to personal care, community-based care, or care coordination.
