Provide RN support for members receiving self-directed personal care through the IRIS program, helping assess needs, coordinate services, and monitor ongoing care. Key Responsibilities
- Conduct nursing assessments for members enrolled in the IRIS self-directed personal care program.
- Coordinate care planning and service needs with members, caregivers, and support teams.
- Monitor member status and help address changes in care needs over time.
- Provide guidance related to personal care services and community-based support options.
- Participate in ongoing communication with program staff and other care partners.
- Document assessments, plans, and care-related follow-up in program records.
Required
- Active RN licensure.
- Experience or training in nursing assessment and care coordination.
- Familiarity with community-based or home-based care settings.
