Key Responsibilities
- Conduct nursing assessments for members receiving self-directed personal care services.
- Coordinate care plans and support service delivery based on individual member needs.
- Monitor ongoing care needs and help address changes in condition or support requirements.
- Collaborate with members, families, and care teams to support safe, effective home- and community-based care.
- Document assessments, care coordination activities, and member progress.
Required
- Registered Nurse (RN) licensure.
- Experience or background in nursing care coordination or personal care services.
- Ability to work with community-based, member-directed care programs.
