This role supports members in a self-directed personal care program, with the RN focusing on care coordination, assessment, and ongoing clinical oversight. Key Responsibilities
- Conduct nursing assessments and ongoing clinical reviews for members receiving personal care support.
- Coordinate care planning and service delivery with members, families, and support teams.
- Monitor member status and identify changes in needs, safety, or care appropriateness.
- Provide guidance related to self-directed care processes and available supports.
- Document assessments, care coordination activities, and member outcomes.
- Participate in collaboration with interdisciplinary or community-based resources as needed.
Required
- Registered Nurse (RN) license.
- Experience in nursing assessment, care coordination, or community-based care.
- Ability to work with members receiving personal care services.
Preferred
- Experience with self-directed care or home- and community-based services.
- Familiarity with long-term care, disability services, or case management.
