Key Responsibilities
- Conduct patient assessments in the field and in community-based settings.
- Provide primary and preventive care support for patients with ongoing health needs.
- Coordinate care plans, referrals, and follow-up with other members of the care team.
- Support management of chronic conditions and help close care gaps.
- Document visits, findings, and care coordination activities in the clinical record.
- Participate in collaboration with interdisciplinary and support teams.
Required
- Active Nurse Practitioner licensure.
- Experience providing direct patient care in a clinical setting.
- Ability to work in field-based and community-based care environments.
