Key Responsibilities
- Conduct care management activities for assigned members in a remote setting.
- Assess clinical and psychosocial needs to support appropriate care planning.
- Coordinate services and referrals across providers and community resources.
- Support members in navigating the health system and accessing needed care.
- Document assessments, interventions, and care plans in the appropriate systems.
- Collaborate with interdisciplinary care teams and other stakeholders as needed.
Required
- Current RN licensure.
- Ability to work in a care management role supporting members remotely.
Preferred
- Experience in care management, case management, or utilization review.
- Familiarity with population health or managed care workflows.
