This role provides remote care management for members in the Detroit area, with a focus on coordinating care, assessing needs, and supporting appropriate use of medical and behavioral health services. The clinician helps address gaps in care, connects members with resources, and works across clinical and community settings to support ongoing care needs.
Key Responsibilities
- Conduct care management assessments and develop individualized care plans
- Coordinate services across providers, facilities, and community resources
- Support members with care transitions, follow-up, and gap closure
- Provide education and guidance related to treatment plans and available benefits
- Monitor member progress and update plans based on changing needs
- Document interventions, outcomes, and care coordination activities in the record
- Participate in multidisciplinary communication to support safe, effective care
Required
- Active RN licensure
- Experience in care management, case management, or a related nursing role
- Ability to work remotely and coordinate care for members in the assigned market
