This RN care manager supports members across Omaha and Lincoln with coordinated care management, helping identify needs, develop care plans, and connect patients to appropriate services. The role focuses on member outreach, assessment, and follow-up for a broad range of clinical and social needs in a managed care setting.
Key Responsibilities
- Conduct telephonic or in-person assessments to identify clinical, behavioral, and social needs.
- Develop and update individualized care plans based on member goals and identified risk factors.
- Coordinate services, referrals, and follow-up across providers and community resources.
- Monitor member progress and outreach needs to support ongoing care transitions and engagement.
- Document care management activities and communicate with interdisciplinary team members.
Required
- Current RN licensure.
- Relevant nursing experience in care management, case management, or a related clinical setting.
- Ability to support members in the Omaha and Lincoln, Nebraska service area.
