This role supports members through care management focused on behavioral health and related clinical needs, with an emphasis on coordinating services, assessments, and follow-up across the care continuum. The work may involve collaborating with multidisciplinary teams and helping members connect with appropriate treatment and community resources.
Key Responsibilities
- Conduct care management assessments and develop individualized care plans.
- Provide coordination and follow-up for members with behavioral health and related needs.
- Coordinate services across providers, care teams, and community resources.
- Participate in interdisciplinary collaboration to support member care and transitions.
- Use documentation and case management workflows to track member progress and interventions.
Required
- Active RN license or behavioral health licensure as an LCSW, LMHC, LMFT, or LMSW.
- Relevant clinical experience in care management, behavioral health, or a related setting.
Preferred
- Experience working in managed care or health plan settings.
- Experience coordinating care across multiple service settings or disciplines.
