This role focuses on care management for members needing long-term services and supports, with an emphasis on coordinating services, assessing needs, and helping guide appropriate care plans. The work is primarily remote and is centered on supporting members in the Detroit area across health and social care needs.
Key Responsibilities
- Conduct member assessments and develop individualized care plans for LTSS needs.
- Coordinate medical, behavioral, and community-based services across the care continuum.
- Provide ongoing care management, follow-up, and support to members and caregivers.
- Use documentation and care coordination workflows to track needs, interventions, and outcomes.
- Participate in collaboration with internal teams and external providers to support continuity of care.
Required
- Current RN licensure.
- Experience in care management, case management, or long-term services and supports.
- Ability to work remotely and support members in the Detroit, MI area.
Preferred
- Experience working with Medicaid or managed care populations.
- Background in community-based or post-acute care coordination.
