Provide care management for adults enrolled in long-term services and supports, with a focus on coordinating resources, assessing needs, and helping members access appropriate services. This role centers on RN-led case management work and may involve collaboration with community providers and other care team members to support safe, effective care plans.
Key Responsibilities
- Conduct member assessments to identify health, functional, and service needs.
- Coordinate long-term services and supports across community and clinical resources.
- Develop and monitor individualized care plans based on member needs and goals.
- Provide ongoing case management and follow-up to support care plan implementation.
- Collaborate with providers, caregivers, and care team partners to address barriers to care.
- Document assessments, interventions, and care coordination activities in the clinical record.
Required
- Registered Nurse (RN) licensure.
- Experience in care management, case management, or a related clinical coordination role.
