Key Responsibilities
- Conduct care management assessments for members with long-term services and supports needs.
- Coordinate individualized care plans and ongoing follow-up based on member needs.
- Provide field-based support and travel within the assigned county.
- Collaborate with interdisciplinary team members and community resources.
- Identify and help arrange appropriate services, supports, and referrals.
- Document care activities and member progress in the care management workflow.
Required
- Active Registered Nurse (RN) licensure.
- Experience or background in care management or long-term services and supports.
- Ability to perform field travel within the assigned service area.
Preferred
- Experience working with complex, community-based, or long-term care populations.
- Prior case management or population health experience.
