This role focuses on managing long-term services and supports for members in the community, with an emphasis on RN care management and in-person field visits in Milwaukee County. The clinician supports members with complex needs by assessing services, coordinating care plans, and connecting patients with appropriate community resources. Work is largely field-based and member-facing, with collaboration across care teams and service providers.
Key Responsibilities
- Conduct field-based assessments and care management visits for members receiving long-term services and supports.
- Coordinate individualized care plans and support ongoing service authorizations and transitions of care.
- Provide care coordination with members, families, providers, and community resources.
- Use clinical judgment to identify member needs, barriers to care, and appropriate supports.
- Document assessments, interventions, and care plans in accordance with organizational and regulatory requirements.
- Participate in interdisciplinary care management activities and collaboration with the broader care team.
Required
- Registered Nurse (RN) licensure.
- Experience in care management, case management, or a related clinical coordination role.
- Ability to travel for field visits in the community.
