The care manager supports members with long-term services and supports needs through field-based nursing assessment, care planning, and ongoing care coordination. Work is centered on helping members access appropriate services, monitor needs over time, and connect with community resources in the Milwaukee County area. This role is primarily field travel-based and is best suited for an RN who is comfortable working independently in the community.
Key Responsibilities
- Conduct member assessments in the field to evaluate health status, functional needs, and service requirements.
- Develop and coordinate individualized care plans for members needing long-term services and supports.
- Coordinate access to community resources, behavioral health supports, and other covered services as needed.
- Monitor member progress and update care plans based on changing needs and outcomes.
- Communicate with members, families, providers, and care teams to support continuity of care.
- Document assessments, interventions, and care coordination activities in the appropriate systems.
Required
- Current RN licensure.
- Experience in nursing, care management, case management, or a related care coordination role.
- Ability to complete field travel within Milwaukee County, WI.
