This role focuses on RN-led care management for members receiving long-term services and supports (LTSS), with work centered on assessing needs, coordinating services, and helping members access appropriate care. The care manager supports members across Boone, DeKalb, Kendall, and LaSalle counties and may work with a broader set of Illinois locations as needed.
Key Responsibilities
- Conduct assessments to identify member needs, care gaps, and service eligibility within the LTSS population.
- Coordinate care plans and community-based services across providers, facilities, and support programs.
- Provide ongoing case management, follow-up, and member support to promote continuity of care.
- Collaborate with interdisciplinary teams and external partners to address medical and social needs.
- Document care activities, service plans, and member outcomes in required systems.
Required
- Registered Nurse (RN) licensure.
- Experience in care management, case management, or a related nursing role.
- Experience working with long-term services and supports or a similar complex care population.
Preferred
- Experience working with Medicaid or managed care populations.
- Experience coordinating community resources and post-acute services.
- Familiarity with interdisciplinary care planning.
