Key Responsibilities
- Conduct care management assessments for members needing long-term services and supports.
- Coordinate service plans and support access to appropriate community-based resources.

- Document assessments, interventions, and follow-up activities in the care management record.
Required
- Current RN license.
- Experience in care management or a related nursing role.
- Experience working with long-term services and supports or a similar population.
