The clinician provides primary care for older adults in skilled nursing and long-term care settings, focusing on assessment, diagnosis, treatment, and chronic disease management. Day-to-day work includes evaluating physical and mental health needs, ordering and interpreting tests, prescribing medications, and adjusting care plans based on ongoing resident progress. The role emphasizes person-centered care and close coordination with an interdisciplinary team.
Key Responsibilities
- Provide comprehensive primary care services to residents in long-term care facilities.
- Conduct patient assessments, including medical history reviews and physical examinations.
- Assess, diagnose, and treat physical and mental health conditions.
- Order and interpret diagnostic tests.
- Develop diagnoses, treatment plans, and medication regimens.
- Prescribe medications and administer treatments as needed.
- Coordinate care with the interdisciplinary team and other healthcare professionals.
- Manage chronic conditions, monitor progress, and adjust care plans.
Required
- Current Nurse Practitioner or Physician Assistant license.
- DEA license.
- At least 6 months of experience in long-term care.
